Telehealth Consent

Telehealth involves the use of electronic communications to enable healthcare providers at different locations to share patient medical information for clinical evaluation, diagnosis, consultation, treatment, and follow-up.

• Delays in evaluation or treatment due to technology failures
• Security breaches despite reasonable safeguards
• Inability to perform a hands-on physical examination

• Improved access to medical care
• More convenient evaluation and management
• Reduced travel time and exposure
• Continuity of care between in-person visits

Telehealth interactions are conducted over encrypted, HIPAA-compliant platforms. No part of the session is recorded without your express written authorization.

All PHI exchanged during telehealth visits is protected under HIPAA and our Notice of Privacy Practices.

Telehealth is not for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately.

You consent to receive electronic communications (email, SMS, secure messaging) related to scheduling, care coordination, and follow-up.

You consent to medical evaluation and treatment recommendations made by licensed VitaFlow clinicians via telehealth, with the understanding that you may refuse any recommendation at any time.

By checking the consent box during scheduling or intake, you provide a legally binding electronic signature equivalent to a handwritten signature under the Florida Electronic Signature Act and the federal E-SIGN Act.

You acknowledge that you have read and understood this consent, have had the opportunity to ask questions, and voluntarily agree to participate in telehealth services with VitaFlow.
